Uttam Associate - i am not satisfied with our service provider service

Dear sir,
We are receiving salary from Uttam associate from 01st March 2009 and our ESIC deducted regularly but till date we have not received ESIC Card or any other Communication regarding ESIC.We have already submmited all required documents. Here i sending details.

Date of Joining :- 01/03/2009
ESIC deducted from Salary is Rs. 149/-in Every month.
Kindly Look in to the matter and oblige us. Awaiting an eraly reply.
