PF settlement - Form 19 claim rejected - Need your help!

Dear Sir / Madam,

My Name is Ranjith D.C and worked for Honeywell from 19-April-2010 to 2-July-2012. Today I checked my PF claim status and came to know that my Form 19 has been rejected again though I have sumbitted the re-authorization form as per Honeywell PF-Helpdesk advise. I am very worried and do not know the reason for this rejection. It has been almost 6 months I have been waiting and still my PF-settlement is not done. Also the strange part is Claim Form 10C has been approved and I have received the payment and I am not sure why your not approving my Form 19 claim. I would really appeciate if you can get this approved at the earliest. Below is the status which I have found in the PF-organization website for your reference.

I have sent couple of emails to PF-office regarding my claim status earlier and have not received any response. I would really appreciate if this issue can be address immediately and process my payment.

Claim ID : PYBOM120********8
Status : Claim Form 10C for Withdrawal Benefit AGAINST Member Account No (PYBOM001********009669) has been approved. Payment is under process.

Status : Claim Form 19 for PF Settlement AGAINST Member Account No (PYBOM001********009669) has been rejected. Rejection letter is under dispatch/ dispatched to address in your claim form.

Please Help, I have applied for my PF at from last three Month.
But there is no responce getting from PF office now below reasons are showing in PF online status. This is one of the intelegent answer given by government office.
Below is the description get from PF online office.
