MOBILE - Iphone 5s screen blackout some time

I purchased iphone 5s 16 gb space gray invoice no. 660 on 12/10/2014 from zenica lifestyle pvt.ltd. gip mall noida up.
IMEI no. 3520440********.
But as on august 2015 the screen has been blackout some time very frequently.plz replace my phone as its in warranty. I request apple india to look in to the matter.
