sir i w as submitted a form to bank of baroda branch gangapur city to issue a barodda jeevan suraksha policy in favour of my wife vandana agrawal and premium amount was deducted by the branch rs 690 from my ac saving bank account with name keshav kumar gupta on 25 january 2011. the amount was deducted by bank on rs. 125 per lac with service charges rs. 13 = 138 into 5 = 690 for sum assured of rs. 5 lac. within one month bank not intimate me for any letter in this matter. on 24 february i was submitted an application to branch to issue a policy in favour of my wife. but no action was made by branch. after passing on 6 month branch inform me through phoe call that policy can not be issued in my wife favour. policy can ibe ssue only in my favour as keshav kumar gupta. the form was taken by me again with detail on 31 july 2011 but sum assured was shown only of rs 200000 as risk cover. with premium amount rs. 533 indian rupee.

in demading clearification of diffrence recovery amount bank branch denied to take application to correct the mistake made by bank branch bankof baroda gangapur city.
