My husband and I were travelling from Aggarwal's Eye hospital ,Whitefield to our residence in munnekolala laxmi layout 2nd cross by an OLA cab booked through his phone.
I lost my mobile phone with sim number ********04 , A LG-D802 model in the cab. The cab details are as follows:

I have lost my stay visa at koramangala 7th block inside the auto while traveling my house to christ University at that time i have lost my original Residential Permit. i hereby declare that now i have applied for duplicate Rp the concerned authourity wants police acknowledgement towards loss of above document the honorable police authority please issue acknowledgment.
