Dear sir,

Reffering above subject please find details below which are related with complaint.

Two Policy Holder Name : 1) Kumud Intodia w.e f. 14/4/2008, Premium Rs.15000/-pa
: 2) Arun Harkawat w.e.f 15/4/2008, Premium Rs.15000/-pa

Proposer name for both policy : Kumud Intodia
Kotak Branch At Udaipur

Adress : 458,Bhupalpura,Udaipur(Rajasthan),313001

Policy Surrender After 3 Years of first Premium & Surrender Dt. At Fatehpura, Udaipur Branch on 16/4/2011.

Total Premium Deposited : Rs.90,000/-

Approx Surrender Policy Value on 16/4/2011 :,Rs. 69,000/-

Kotak Branch Udaipur was Committed to Deposit of surrender value/amt. in Given Bank Account : Within 10 Days i.e. 27/4/2011

Complaint dt. ; 05/05/2011

Please let me know the status of above with reply on same mail,immediately.


Also,please correct the date in your online complaint form dt.5/5/2011 for above Policies surrender dt written 16/3.2011 instead of 16/4/2011.

Note: Somebody is calling from 0091-1140409393 on my mobile and telling that he is speaking from Kotak life insurance,Delhi and when again
I called back for above matter enquiry to same above Delhi telp. no. then Mr.Ritesh giving me reply that we are from Bajaj Allienz.

With reference to your Policy# 997908, we would like to confirm that the surrender proceeds were credited to your Bank Of India account on 2 May 2011.

Regarding P# 1002632, we would like to confirm that the surrender proceeds were credited to your Bank Of India account on 29 April 2011.
