Dear Sir,

I received a SMS on dated 26th August 2011 at 4.00 p.m one transaction was made on my card amounting of Rs. 4964.00. I didn’t have any idea about the transaction. Immediately I called the customer care (Mr. Kartik) and asked him to block the card I think he has done it. He suggested sending the dispute form immediately.

I am working with CBI department, Bhubaneswar, Orissa and holding Standard Charterd and SBI Card but till date I never face this type of problem. How this has happened with Axis bank card and surprisingly since 2009 I have not used this card.

In this regard, I am not going to make any sort of payment in lieu of this transaction. And would request you to please pursue the matter with the concerned party and take appropriate legal action or I will ask my department officials to pursue the matter with Mumbai officials (CBI). Meanwhile, I am also filing a case in consumer forum.

Herewith, I am enclosing filled-up dispute form duly signed by me for your reference and necessary action.
