On 7th April I contacted ICICI bank to make an FD they transferred my call to ICICI pru. Finally at the end of the call they deducted 99900 online from my kids account and made a Life insurance for me. I had asked them for investment in the name of my son. When I saw the details of the application, I asked for cancellation. They acknowledged my request. But without me having signed the application or giving the supporting documents they have issued a life insurance policy in my name on 13-April-2011. Repeated emails to Level 1 & escalation to Level 2 is not helping.
Application no. is OL00085697. Policy no 15357778
Yesterday, 17th April I received a soft copy of my Policy.

Complainant's Goal: Immediate cancellation of my application and complete refund of my money.
